Service Description

This Includes short and long-term supports that focus on Participants’ individual needs, on strengthening Participants’ choices and abilities to coordinate their supports, and to assist them to live at home, maintain their independence, and participate in their community. • Assistance with decision making, daily planning, budgeting, life planning, and goal planning: • Coordination of Supports: • Individual Life skill development: • Assistance with accommodation and tenancy obligations. • Life transition planning including mentoring, peer support. • Development of independent self-care and living skills. • Establish supportive networks in a Participant’s community circle. • Collaborative arrangements with other community services and programs.
